Diet Plan

Occupational Therapy

It is a cohesive component for addressing wholistic rehabilitation through meshing the medical model with client-centered goals to help achieve a desired level of independence. Occupation Therapy is dedicated to the restoration, adaptation, and finding the awesome purpose through this life-changing experience. The areas addressed include: 

  • Optimal Utilization of adaptive measures and equipment.

  • Strengthening through exercise.

  • Community re-integration training.

  • Building endurance through time.

  • Addressing activities of daily living (ADL) and instrumental activities of daily living.

  • Cognitive Training

  • Diagnosis-specific education regarding co-morbidity and A/D abuse

Psychotherapy

Nursing Servivces

The Psychotherapists of Customized Medical Needs have both psychological and psychiatric training but tend to concentrate on the counseling and behavior modification aspects when it comes to the treatment of our clients. The Psychotherapist will generally act as a guide and mentor to our clients while the client works through stressful or self-limiting personal issues.

Skilled Nursing is the care provided by our Family Nurse Practitioner (FNP) in our clinic. Our service includes  Wellness visits, physicals, pain management, allergy testing, check-ups, etc.

 

Home health episodes are handled by a  Registered Nurse (RN) or Licensed Practical Nurse (LPN) in accordance with a physician's signed plan of treatment in the comfort and security of a patient's home.

The RN makes the initial evaluation visit and establishes a nursing Plan of Care (POC) for the patient. The RN regularly reevaluates the nursing needs of the client and makes any necessary adjustments. The RN coordinates the total POC including teaching the patient and/or caregiver about the client's care.

Chronic Care Management

Chronic Care Management (CCM) is defined as non- face-to-face services provided to Medicare (and other Government/Commercial Insurance) beneficiaries who have multiple (two or more), significant chronic conditions that will last for at least 12 months or the remainder of the patient's life.

 

CMN can provide proactive, structured, and regularly scheduled interaction with your chronically ill patients. The offload of these activities will help you achieve better results at a lower cost, meets the new CMS guidelines with reimbursement of up to  $40.09 per patient per month; delivering better care coordination with a complete view of your patients!

Registered Dietitian

At Customized Medical Needs, our Registered Dietitians evaluate and design a customized nutritional plan that can enhance your health or the health of your loved one, based upon dietary needs, medical conditions, and physician recommendations. We cover complete health & wellness as well as macronutrients.

Auricular-Therapy

An important component of the rehabilitation program. It helps clients through the various changes that occur in the body in the rehabilitation process. In some instances, the withdrawal symptoms can be quite heavily tasking on the body. Auricular Therapy helps to address this through the complete detoxification of the body and helping to calm the nerves.
